MyChart proxy access allows a parent or legal guardian to access their family member’s useful medical chart information. Parents can utilize MyChart to access their children’s information, schedule an appointment, print immunization records and more. MyChart account holders may also help manage information for an aging parent or relative with help in scheduling appointments, reviewing hospital discharge information or monitoring medications.
What access is a MyChart proxy given?
Access varies based on the age of the patient.
- Parent-Child: this provides access to your minor child's MyChart record up to age 18.
- Adult-Adult: this provides access to another adult's MyChart record with the authorization of the subject (with exceptions).
- Legal Guardian: this provides access to the MyChart account of a patient for whom you have documented legal authority to make healthcare decisions. Proof of legal guardianship is required.
How do I request MyChart proxy access?
Proxy Access for Current MyChart Users:
- For minor proxy access, current MyChart users can request minor child and legal guardian access from within MyChart. Simply log in to your MyChart account and fill out the "Request Minor Proxy Access" form found in the MyChart menu.
- For adult proxy access, current MyChart users can grant other adult MyChart users access to their medical records using MyChart's "Proxy Invite" feature. This can be found within the "Sharing Hub."
Proxy Access for Those Without a MyChart Account:
- Complete and submit one of the printable MyChart proxy forms provided below and return it to the patient’s Warren Clinic physician office or the Saint Francis Health Information Management department (address is on the form) for review and approval.

What happens next?
Proxy Access for Current MyChart Users:
- Once an online proxy form is submitted, proxy access is enabled within five to seven business days. Please be sure to fill in everything on the form and use the patient's exact name. Missing or questionable information will result in a follow up email from our team and could lead to a delay. Notification of proxy activation will be sent to the parent’s/caregiver’s MyChart account.
Proxy Access for Those Without a MyChart Account:
- Once a proxy consent form is submitted and approved, the parent/caregiver will be given a non-patient MyChart account. This account will be linked to the patient’s MyChart account for access. Be sure the form is complete, and the patient's exact name is listed before submitting the form. Please allow at least seven business days for processing of a manual proxy request. Any missing information will result in a delay. Notification of proxy activation and instructions to access MyChart will be sent to the parent’s/caregiver’s email address
