What is MyChart?

MyChart is a secure, online tool that allows patients to connect with their personal health information 24/7. Patients can access MyChart from any Internet connected computer, tablet or smartphone device.

With MyChart you can...

How do I sign up?

Signing up for a MyChart account is easy with one of our many signup options:

Activation Links

  1. Sign up with a convenient link sent via email or text message received when you check-in for an appointment or when you are admitted or discharged from the hospital.
  2. Open the link and follow the instructions to confirm your identity and set up a username and password.

Activation Codes

  1. Activation codes are printed on After Visit Summaries, letters and billing statements.
  2. On the MyChart login page, click “Sign Up Now” and enter your activation code and required information. Then follow the prompts to create your account.

Direct Signup

  1. If you don’t have a link or a code, you can still sign up by entering your information to match to your patient records.
  2. On the MyChart login page, click “Sign Up Now” and then “Sign Up Online” to access a demographic information form. Complete the form and follow the prompts to confirm your identity and create your account.

What technology do I need to log into MyChart? 
You need access to a computer, tablet or other mobile device connected to the Internet and an up-to-date browser or the MyChart app.

Is there a fee to use MyChart?

MyChart is a free service offered to Saint Francis Health System patients.

Is MyChart secure?

Access to information is controlled through secure activation codes and links, Two-Factor authentication, usernames and passwords. Each patient controls the account’s password, and the account cannot be accessed without that password. MyChart uses the latest encryption technology to secure the integrity of your account. Unlike conventional email, all MyChart messaging is done while you are securely logged on to the website.

MyChart also uses Two-Factor Authentication by default, sending a security code to your verified email or phone number to authorize any new logins to your account.

Which physicians may I make appointments with in MyChart?

You can use MyChart to schedule appointments with physicians you’ve seen in the recent past. You can also use MyChart to seek care with a new primary care physician by visiting our Provider Finder and searching for providers allowing online scheduling for new patients.

What is eCheck-In?

eCheck-In allows you to check in online before a scheduled outpatient visit and reduces the need for completing additional paperwork during your visit. You can submit health history information, review and suggest changes to your medications and allergies, make copays, answer questionnaires, and sign documents electronically in MyChart to save yourself time at check-in.

Two days prior to your appointment, you will receive an email or text reminder to complete eCheck-In.

How can I change my notification preferences?

Change your notification preferences, including which notifications you receive and how you receive them, by visiting your “Communication Preferences” page.

How do I sign up for Text Messages about Appointments and other information in my record?

You can sign up for text messaging by visiting your Communication Preferences page and selecting the Opt-In option under Text Messaging. You can also simply text “START” to 28637 to start receiving texts. The staff at your next appointment can also set up your text messaging preferences.

Text messages related to your relationship with Saint Francis HealthLink, including updates related to your visits, MyChart account, one-time passcode, billing notifications, prescription reminders, and care management will be sent to the mobile number subscribed. Message and data rates may apply. Message frequency may vary. For help text HELP and text STOP to opt out of notifications from a specific short code.

How do I receive appointment offers when I'm on the wait list?

When you’re on the wait list, you’ll automatically receive text message or email notifications when an earlier appointment slot opens. Automatic waitlist offers can be sent up to five patients at once, so it’s important to respond by replying to your text message or signing into your MyChart account to review any offers at your earliest convenience. If you miss an offer, don’t worry – you’ll keep your place on the wait list for additional offers in the future. You can easily add yourself to the wait list by looking at any future appointment on your Visits page.

When can I see my test results in MyChart?

Saint Francis MyChart allows you to see lab test results and imaging reports at the same time they are released to your provider. This includes access to laboratory reports, pathology reports, genetic testing information and more.

If I send a message to my physician or nurse, when can I expect a reply?

You will generally receive an answer within one to three business days. Please note that MyChart should not be used for urgent situations. Please dial 911 if it is an emergency.

Will my entire medical history be made available in MyChart?

MyChart is a direct link between you and the information in your electronic medical record. It will not reflect your complete medical history, though some patients may see past records in order to ensure continuity of care and access to information about ongoing treatments.

Past appointments in your electronic medical record can be viewed under Visits > Past Appointments.

Can I ask my primary care physician for medical advice through MyChart?

Yes. You can send a secure message to your Warren Clinic or Laureate physician if you have seen them within the last three years. Use the “Ask a Question” link in your Menu to write a message to your physician.

Your messages are triaged by your physician’s nurse or medical assistant to help your physician more quickly address your questions and medical concerns.

Can I view a family member’s health record in MyChart?

Yes, this is called proxy access and allows a parent (or legal guardian) to log into their personal MyChart account, and then connect to information regarding their child and/or teen. Adult patient information may be accessed by family members with permission of the patient, or if the patient is incapacitated. Guardians, powers of attorney or other legally authorized representatives may be allowed to access information of the patient on MyChart. Can I ask questions regarding a family member from my MyChart account?

MyChart offers direct access to your personal health record. If you ask a question about a family member using your own account, the response would appear in your health record and not in the family member’s health record, potentially jeopardizing your family member’s medical care. We recommend asking questions only about the patient who is currently logged in to MyChart or as a proxy on behalf of the family member while accessing their MyChart account

Can my spouse and I share one MyChart account?

No, each adult must establish their own MyChart account. Spouses can establish proxy access once both have their own account.

If some of my health information on MyChart is not correct, what should I do?

Your MyChart information comes directly from your electronic medical record at your physician’s office or the hospital. Contact your physician’s office to correct any inaccurate information at your next clinic visit. Your health information is reviewed and updated in your electronic medical record each visit.

Is my activation code my user ID?

No, your activation code is not your MyChart ID or password. You will use this code only once to log into MyChart for the first time. When you log into MyChart the first time, you will then be asked to create your own unique MyChart ID and password.

Can you send me a new activation code as I have lost it or did not receive it?

Yes. Please Contact your Warren Clinic or Laureate physician’s office to request an activation link, which will be sent to your email or mobile number.

My activation code does not work; what should I do?

Please contact your Warren Clinic or Laureate physician’s office for assistance. You may need a new code or link to sign up or your contact information may be out of date.

Where can I update my personal information (e.g., home address, email or change my password)?

Log in to MyChart and go to the “Personal Information” page. Click or tap “Edit” on the appropriate section to edit the information.

I forgot my password. What should I do?

Visit the Saint Francis MyChart site and click the "Forgot Password" link on the sign-in page to reset your password online.

What is MyChart’s Privacy Policy?

MyChart is fully compliant with federal and state laws pertaining to your privacy. Your name and email address will be treated with the same care and privacy given to your health records.
