Comprehensive Primary Care Initiative
Selected Warren Clinic primary care offices participate in the Comprehensive Primary Care Initiative (CPCI). By partnering with Medicare, Medicaid, CommunityCare and Blue Cross Blue Shield, these offices work together with their patients to better coordinate care visits and create a care plan to meet the patient’s unique health goals.
What Does the CPCI Mean to Patients?
- Participants have a physician-directed patient-centered care team. This team includes the primary care doctor; other healthcare professionals, including a registered Care Guidance nurse; the patient’s family (if the patient desires) and the patient.
- A patient's healthcare is coordinated, comprehensive and evidence-based. In addition to care for illness and injuries, each patient has a personalized care plan that includes preventive health services. Should specialty care be required, the team works to coordinate with the specialists or other healthcare providers.
- Quality and safety are priorities. CPCI offices measure their performance to find ways to continually improve care.
- Patient survey responses guide our development and monitoring of improvement plans.
- CPCI offices strive to make it easier for patients to communicate with their care team and to ensure that appointments are available with the patient’s personal physician.
- Consistent with our Health System's value of Stewardship, we ensure prudent use of talents and resources in coordinating care.
