About Us
Healthy Oklahoma Care Network (HOCN) is a clinically integrated network (CIN) composed of Saint Francis Health System, Warren Clinic and carefully selected provider organizations throughout eastern Oklahoma. HOCN and its participating providers strive to enhance the quality of care, reduce costs and improve health outcomes for patients. When we are successful in these endeavors, our payor relationships reward our participating providers.
Our vision is to create a healthier population – one person at a time – by offering a network of providers committed to comprehensive, data-driven and coordinated care. Our network currently provides care to over 100,000 patients under value-based care arrangements.
Why Consider Joining HOCN?
- Eligible for quality incentives and shared savings distributions contingent upon the successful performance of HOCN value-based agreements. PCPs may access care coordination payments to invest in care coordination support resources.
- Access to patients insured in HOCN-exclusive insurance products and HOCN’s payor agreements.
- Access to Population Health Management resources and programs for patients covered under HOCN’s value-based agreements. These include resources to support patients in the following ways:
- Addressing gaps in care (i.e. quality measures)
- Overcoming social determinants of health
- Managing of chronic conditions
- Transitioning levels of care
- Assisting in medication access
- Access to Population Health technology tools to identify patients with gaps in care or in need of preventative care measures and support providers’ efforts to accurately code the conditions that impact a patient’s health status.
