Saint Francis Health System physicians and staff are dedicated to the health of patients even after leaving the hospital. Many services are available to help patients continue to heal outside of the hospital setting. Following the doctor's discharge plan will go a long way to ensure recovery is the best it can be.
Hospital Discharge
Once your physician has determined you are well enough to leave the hospital, they will submit a discharge order. There are a couple of items you can do to make your discharge easier:
- When possible, send flowers and/or gifts as well as extra clothing or personal items home the day before.
- Discuss billing arrangements with a financial counselor to help ensure everything is correct before leaving.
When the doctor has notified you that you may leave the hospital the nurse will begin the discharge process. Generally, patients are released from the hospital before noon.
Post-Hospital Care
Home Health Care
Saint Francis Home Health and All Saints Home Medical Equipment work together to provide the equipment and staff needed for additional care at home. Talk with your discharge nurse about next steps to ensure everything needed at home is set up before leaving the hospital.
Cardiac and Physical Rehabilitation
Physical medicine specialists from Saint Francis work to restore physical strength, cognition and mobility to patients recovering from a number of conditions. Cardiac Rehab and Physical Medicine (including occupational, physical and/or speech-language therapy) may be needed to increase the patient's independence and restore self-sufficiency.
Skilled Nursing
For our Grand Lake area patients, Saint Francis Hospital Vinita offers a "swing bed" program. Geared for patients who are ready to leave the hospital, but who are not yet strong enough to return home, this short-term program helps patients transition between their hospital stay and returning home - all while staying close to home and their support system.
Routine Healthcare
One of the best ways to stay out of the hospital is to develop a habit of routine checkups. A relationship with a primary care provider can go a long way to identify problem areas early and take steps to prevent further damage - sometimes even reversing the condition.
