Medicare Advantage FAQs for Patients

Saint Francis and Warren Clinic are committed to delivering high-quality, affordable healthcare to our patients. 

Beginning January 1, 2025, Warren Clinic’s primary care offices will require Medicare patients to be enrolled with a Medicare Advantage plan where Warren Clinic is an in-network provider or have Traditional Medicare coverage.

IMPORTANT: This change does not affect your coverage with any Warren Clinic specialist or any other Saint Francis services – you may continue to access all specialty care and hospital services under any Medicare coverage.

What can I do?

With Medicare open enrollment coming up October 15 - December 7, 2024, you have time to make a change in your Medicare Advantage plan for next year.

How do I make the change?

If you prefer to stay with your Warren Clinic primary care physician, you may opt to switch your Medicare Advantage plan. You can make the change during the annual Medicare open enrollment period between October 15 – December 7.

Non-Contracted Medicare Advantage Health Plan – Patient Continuity of Care Request

We recognize that some patients may have an employer-sponsored Medicare Advantage retiree plan and are unable to adjust their coverage without a financial penalty. For patients in these circumstances, Warren Clinic has an exception process that will allow you to maintain your Warren Clinic Primary Care Provider.

We respect that you need continuity of care. If you have an employer-sponsored Medicare Advantage retiree plan, you may submit a request for an exception using the form linked below:
