Refer a Patient

Every patient’s circumstances are as unique as the individuals behind them. The decision and process of referring a patient to a higher level of care may at times be difficult for sufferers, families and caregivers. Our admissions team hopes to partner with individuals, families and eating disorders clinicians to provide seamless communication and access to information when navigating the referral process.

Referral Process

  1. Call Laureate and speak to one of our admissions coordinators. They will gather demographic information, ask preliminary questions, obtain clinical information and gather insurance information to verify benefits. | 918-491-5775 or 800-322-5173
  2. The initial clinical and insurance information will be taken to the treatment team for approval and to estimate the length of stay.
  3. Your admissions coordinator will then call you to review the information. At this point, the patient will go on our waiting list. Once an admissions date becomes available, the admissions team will call the adult patient or adolescent caregiver to offer an admission date.

Before Admission

About a week before admission to the program, the admissions coordinator will schedule a phone assessment with the adult patient or parent(s) of the adolescent patient and gather information from the current outpatient treatment team.

  • The patient/parent phone assessment is typically done with one or both parents and takes about an hour and a half. It is scheduled in advance about a week before admission, or sooner if needed.

We will need labs and an EKG that have been completed within the past 30 days. Required tests include:

  • Chemistry Panel 14
  • Complete blood count (CBC) with differential
  • Prealbumin
  • Urinalysis (UA)
  • EKG

If you have labs that have been completed outside the 30-day window, please send them as well; they can be used for comparison. We have found that it is easier to sign the release of information (ROI) at the time of the test so that results can be sent to us from the lab as soon as they are obtained.

Finally, Laureate will e-mail information regarding admission, such as a list of what to bring, etc. to the patient or adolescent patient's parents.
