Medical Benefits

Dental Insurance Plan
- Those eligible: full-time employees
- Immediate upon employment**
- Dental coverage for employee and eligible dependents. Orthodontic coverage for children under the age of 19.
TaxSaver Plan
- Those eligible: full-time and part-time employees
- Immediate upon employment**
- Allow Medical and Dental premiums to be deducted Pre-Tax
- Medical Spending Account – Ability to pay out-of-pocket health/dental expenses (expenses must be eligible under section 125 guidelines) that are not covered by medical, dental or vision plans with pre-tax dollars.
- Daycare Spending Account – Ability to pay section 125 eligible daycare expenses that are incurred while working with pre-taxed dollars.
- Election amounts for each of the accounts are a minimum of $260 and a maximum of $5,000 per year.
- Deductions are withheld in equal installments throughout the year.
- Reimbursements received are non-taxable.
- This is a Use-It-or-Lose It program.
- Contribution amount must be re-elected each year during Open Enrollment for the following calendar year.
Life Insurance
- Those eligible: full-time employees
- Immediate upon full time employment
- SFHS will provide 1x base annual earnings in Basic Life insurance and Accidental Death/Dismemberment.
Optional Term Life Insurance
- Those eligible: full-time employees
- Immediate upon full time employment**
- Employees may purchase an additional one, two, three, four, five or six times base annual earnings life insurance.

Dependent Life Insurance
- Those eligible: full-time employees
- Immediate upon full-time employment**.
- Employees may purchase dependent life insurance equal to $10,000 for spouse/ $5,000 for children over 6 months and $200 for children under 6 months.

Basic Retirement Plan
- Those eligible: full-time, part-time, and ECB employees
- Eligible after 12 months of employment, paid 1,000 hours, and age 21
- SFHS matches 6% of salary and contributes it to a retirement account
401k Plan
- Those eligible: full-time, part-time and ECB employees
- Individual pre-tax retirement savings
- Immediate upon hire
- Employees are AUTOMATICALLY enrolled in the plan at 3% after your 30 day election period has ended

- Employees may increase/decrease their contributions at any time by calling Fidelity at 1(800)343-0860 or logging onto www.Fidelity.com/atwork.
- After 12 months of employment, working 1,000 paid hours and at least 21 years of age, Saint Francis Health System will match 50 percent of first 8 percent that the employee contributes
PTO
- Those eligible: full-time and part-time employees
- Immediate upon employment
- Paid time off for vacations, personal use, holidays and the first 24 consecutive hours of personal illness or injury.
- May carry over up to 40 days (320 hours) to the next year.
- Full-time employees may accrue up to 21 days (168 hours) during their first through fifth year of service; 26 days (208 hours) during their sixth through 10th year of service; 31 days (248 hours) during their 11th year or more of service.
- Part-time employees may accrue up to 10 days (80 hours) during their first through fifth year of service; 15 days (120 hours) during their sixth through 10th year of service; 20 days (160 hours) during their 11th year or more of service.
Saint Francis Health System recognized Holidays
- New Year’s Day, Good Friday, Memorial Day, Independence Day, Labor Day, Thanksgiving Day and Christmas Day.
- Part-time and ECB employees receive an extra hour of base pay for every hour worked on a holiday.
Extended Sick Leave
- Those eligible: full-time employees
- Immediate upon employment
- Paid time off for personal illness or injury lasting more than 24 consecutive scheduled hours.
- May accrue up to 7 days (56 hours) per year and carry over a total of 1,040 hours for 100 percent of pay to the next year.
Tuition Assistance Plan
- Those eligible: full-time and part-time employees
- After completion of 6 months of employment in an eligible status
- May receive reimbursement for Tuition cost, less any books or fees up to annual maximum levels
- Full-time: $3,000/yr Basic, $4,500 Critical (refer to Tuition Assistance Policy on HealthNet)
- Part-time: $1,500/yr Basic, $3,000 Critical (refer to Tuition Assistance Policy on HeathNet)
- Requires employee to remain in a full-time or part-time working status for 6 months following completion of the course.
- Grades and Tuition Reimbursement Request form must be completed and received no later than 60 days following the course end date to be eligible for reimbursement.
Health Zone
- Those eligible: full-time, part-time and ECB
- Immediate upon employment
- Employee discount for Health Zone membership.
Ave Maria House
- Those eligible: full-time, part-time and ECB

- The Child Development Center is open Monday through Friday 6:20 AM to 8:00 PM and is located on the Saint Francis Hospital campus
- The School Age Program for the Four Year Old program and the Before/After school program operates Monday through Friday 6:00 AM to 6:00 PM and is located at 5710 E 61st Street South.

- The EAP’s professional counselors offer you an assessment of your problem, short-term counseling, information and referral to community resources.
Family and Medical Leave of Absence
- Those eligible: full, part-time and ECB employees
- Eligible after 12 months of employment and 1,250 hours worked prior to leave.
- Up to 12 weeks leave with retention of benefits and job following a temporary absence required due to birth or placement of a child or serious medical condition of employee, spouse, child or parent. (In accordance with policy.)
- Leaves are subject to HR approval by following DOL guidelines.
Medical Leave of Absence
- Those eligible: full-time and part-time employees whom are not eligible for FMLA or have exhausted their FMLA based on leave type
- Retention of accrued benefits. Types of leaves are medical and military. Leaves are subject to HR approval.
** Must enroll within first 30 days of eligibility, coverage becomes effective the day the completed enrollment form is submitted to HR Benefits.
